STALKING
(NON-DOMESTIC RELATIONSHIP)

If you have not done so already, take the time now to read the Special Notice.

In this section, you can download the forms to petition the court for a Personal Protection Order (PPO) Against Stalking (Non-Domestic Relationship). The forms will include instructions on how they should be filled out, and the link below will provide you step-by-step instructions on the entire process.

Remember that you should use PPO forms for Stalking if you want an order to restrain another person from stalking you and you do not have a domestic relationship with this person.

To petition the court to grant for a Personal Protection Order (PPO) for Stalking, you must file form 377 - Petition for Personal Protection Order Against Stalking (Non-Domestic Relationship). If you are in immediate danger or would suffer irreparable injury loss, or damage if there is a delay, you can ask the judge to issue a Personal Protection Order for Stalking without a hearing (in Latin, "ex parte"). To do so, you must file form 377 and check F - ex parte, and, in addition, file form 380 - Personal Protection Order Against Stalking - Ex Parte (Non-Domestic Relationship). Each form is two pages long, and each one comes with two pages of instructions on filling out the form. Again, the link at the bottom of this page will then take you to the step-by-step instructions on the process.

To view and print one or both forms (or save to the forms for printing out later), click on the appropriate form numbers above. (If you choose to save the file, remember that saving it to your hard drive means that anyone else with access to your computer can find the forms filed there. A safer option might be to save them to a diskette that you can keep confidential.) Click here to learn how to clear your computer of any record that you have visited this site.

NOTE: When you print out the forms, make color copies as instructed on the top of the forms:

  • Form CC 377 - Original (white) for the court and one each in green, blue, pink, and yellow.

  • Form CC 380 - Original (white) for the court and one each in green, blue, pink, yellow, and goldenrod.

If you find that you cannot view or save the form, you may need to download Adobe Acrobat Reader. To download a free copy, click on the icon below. Again, read Be Safe Online, if you have questions about the safety of downloading.
