To
petition the court to grant for a Personal Protection Order (PPO) for
Domestic Relationship, you must file form 375 -
Petition for Personal Protection Order (Domestic Relationship).
If you are in immediate danger or would suffer irreparable injury loss,
or damage if there is a delay, you can ask the judge to issue a Personal
Protection Order without a hearing (in Latin, "ex parte"). To do so,
you must file form 375 and check G - ex parte, and, in addition, file
form 376 - Personal Protection Order - Ex Parte
(Domestic Relationship). Each form is two pages long, and each one
comes with two pages of instructions on filling out the form. Again,
the link at the bottom of this page will then take you to the step-by-step
instructions on the process.
